| To order an item from the Buffalo Boutique, complete the form below and print it out. Please make checks payable to the Parents' Association and place this form in an envelope marked BUFFALO BOUTIQUE. Envelopes may be placed directly into the Buffalo Boutique mailbox, sent in with your child, or mailed directly to Old Trail School( 2315 Ira Rd., P. O. Box 827, Bath, OH 44210-0827). Merchandise will be delivered to your child upon receipt of payment. A $2.00 handling fee will be added to any merchandise that needs to be shipped. Please be sure to indicate if this is a gift and needs to be disguised. |
